What you're assuming is wrong however. Nobody has bothered to really go in depth about this game in that aspect. To answer your questions:

Stage doesn't matter. Depends on game mode. Placement when you *get* the item is deciding factor. RNG rates are unknown.

Probably. As a kid I've gone backwards a bunch of times and you'd need to go the same amount of laps forward in order for them to "start counting" again. Has someone tested this cap yet? Nope.

It's about getting the air, not you throwing the blue shell. You could blow yourself up with a bob-omb. If you check out the Special Cup WR you can see a double Bowser Skip, one shell thrown by me, one by CPUs.
