When a player has been frozen and been sent a request for proof because their submissions look suspicious, is there then a procedure in place to set the Frozen players to Removed after a certain period of time?

It looks like there are some submissions that have been Frozen for over 10 years but they still get listed in the rankings. If they have just submitted fake times and haven't been able to provide evidence to back them up then this isn't really fair on legitimate players as it can throw out their 'true' ranking and also the 'true' order of the rankings depending on what the fake track times are.

There's not really a concrete policy. It's mostly up to whichever updater is responsible for a site's news to decide if they want to remove a player entirely.

There's 5 guys with Frozen icons for MKDD and I've rejigged the rankings to automatically place them on the bottom of the AF/ARR/PRSR. This somewhat addresses your concern, since they don't make you lose a rank, yet they are given the opportunity to prove themselves if they want their real rank back. It doesn't fix everything since their individual scores still affect people's AFs by small amounts, but likely not enough to result in changes in rank unless two players are in a dead heat.

Cool, that's probably the best way to do it so they still have opportunity to come back.

I was just thinking if a suspicious frozen player had some track times that were listed between other players that were close overall it could distort the true order of the overall ranking. I'm not sure how else you can resolve that though. I guess there aren't enough of them to worry too much about it and it be a real issue.
